COLLECTION AND USE OF PERSONAL HEALTH INFORMATION:
In order to provide counselling services, Real life Counselling will collect some personal information about you. We may also ask you to complete questionnaires or other self-report tests to enable a better understanding of your mental health and on going support needs.
The primary purpose of collecting this information is so we may:
Evaluate your mental health needs;
Provide appropriate therapy strategies.
Third parties such as ACC Sensitive Claims, when applicable, may request specific information about your status:
ACC and the sensitive claims team, for example, often have your consent for us to release information about session dates, duration and the clinical provider’s information (name, title, registration number); and
ACC or WINZ may request, with your permission, information related to your mental health, support needs, wellbeing plan progress in order to make determinations of your entitlement to funding.
We reserve the right to disclose your personally identifiable information as required by law, when we believe that disclosure is necessary to protect our rights, and/or to comply with a judicial proceeding, court order or other legal process.
